IF PAYING WITH CASH, CHECK, CREDIT/DEBIT CARD, OR MONEY ORDER

You have the option to fill out a paper order form and present proper identification at the Customer Service Window.   If you are not a qualified applicant, you will need to support your entitlement to the record requested at the time your application is submitted or have a qualified applicant submit the application. Order forms are located on the counter to your left as you enter our office. We accept cash, check, credit/debit cards or money orders for our fees at our customer service window.  

Local County Health Department

With the Introduction of our electronic issuance system, all county health departments can issue any birth or death certificates that have been registered statewide with our office. You will no longer have to travel to the county where the birth or death occurred. Identification will be required at the time of submission of your application and fees payable. IF you are not a qualified applicant, (parent, child, or spouse), you will also need to present any documents to support your right to the certificate. Entitlement documents can include custody orders, letters or testamentary, insurance policies denoting the beneficiaries, etc. 

Online Orders

Tennessee Office of Vital Records does not directly accept online orders. However, for your convenience, we have contracted with an independent company to provide you with this service. VitalChek is the only official vendor authorized by the state of Tennessee to process credit / debit card transactions for online orders. Any additional fees will be collected by the independent company.

A birth extract is a certified true copy of the Birth Certificate (BC). Once a birth certificate is lost and a replacement is required, applicant has to apply for a birth extract. All applications for a certified extract of these records have to be made on the requisite application forms. Applications can be made in person or via the Internet.

Eligibility 

  • The BC holder or his parents.
  • Applications made on behalf of relatives or friends must be accompanied with a letter of authorisation stating the name and identification number of the applicant and his/her IC relationship with the BC holder. If the applicant is not holding a Singapore Identity Card, a certified copy of his/her official identification document (e.g. identity card, passport, driver's licence, etc) must also be submitted.

Application in Person 

Kindly contact the Mission for the application form (BD 40) and submit it together with the following required documents:

  • Photocopy of your relevant identification documents i.e. Passport, NRIC, Birth Certificate, Citizenship Certificate (if applicable)
  • A5 size courier bag (for the return of your documents)
  • NZ$44.00 – Fee per Certified Birth Extract (if a copy of Birth Certificate is produced)
  • NZ$44.00 – Search fee per month
  • NZ$88.00 – Total fee for Search (of one month) and Certified Birth Extract

Note:  If the Birth Registration Number is not furnished, applicants are required to pay a search fee.

Please submit your application with the appropriate fees in cash or Bank Cheque made payable to the "Singapore High Commission" (Note: personal and company cheques will not be accepted and fees are subject to change without prior notice).

Application Online

  • Applicant can apply through the Singapore Immigration and Checkpoints Authority (ICA) website at https://extracts.ica.gov.sg.
  • Submit the form electronically and send payment to the Registry of Births & Deaths.
  • The processing time is approximately 1 week upon receipt of payment.

In order to request a copy of a birth certificate you must complete the form Application for Search of Birth Record Files. To learn more, see FORMS located in the right-hand column. Completely fill out the form. Submit it with the following information: 

A valid government issued photo identification (ID) that must be readable and is not expired/out of date (if an ID is not provided, unreadable, or expired, the request will be returned unprocessed).
