False-Negative Pregnancy Tests

At Your Options Medical, we have found that when many of our patients have a negative pregnancy test result, they assume they are not pregnant. However, this is not always the case. False negative pregnancy tests do occur. Here is how it works:

A urine pregnancy test detects the hormone called hCG (Human Chorionic Gonadotropin). This is the hormone your body produces during pregnancy. When urine is being tested for the presence of this hCG hormone and a woman is pregnant, the result of the pregnancy test should read positive. However, there are a few reasons a pregnancy test can give a false test result including the following:

  • In early pregnancy, the hCG hormone levels may be too low for a pregnancy test to detect. The earliest time to test for an accurate result is 7-10 days after a missed period.
  • The best time of day to take a pregnancy test is first thing in the morning when a woman’s urine is highly concentrated. Because urine may be less concentrated later in the day, the test may not be able to detect the hCG hormone.
  • If you have had a negative pregnancy test, but are experiencing pregnancy-like symptoms such as nausea, vomiting, tiredness, loss of appetite, etc. it is always best to contact your doctor to access for causes unrelated to pregnancy.
  • There is always the possibility of human error when performing home pregnancy tests. It is important to follow the manufacturer’s instructions. Most pregnancy tests require you to let the test set for 3-5 minutes. If the test is left beyond the recommended set time, this could produce a false test result.
  • If you had a tubal ligation (i.e. tubes tied), are experiencing pregnancy-like symptoms, and test negative on a pregnancy test, contact your medical doctor to assess for causes unrelated to pregnancy.

If you are reading this article, you are probably facing the well-known dilemma: "Am I pregnant or not?" You may be wondering why you're experiencing early pregnancy symptoms like nausea, tender breasts, minor cramps, or fatigue, but when you took a pregnancy test, the result was negative. We will discuss the various reasons why a pregnancy test came out negative but you feel pregnant.

Causes of Pregnancy Symptoms With a Negative Test

  • You are experiencing premenstrual symptoms, which are very similar to early pregnancy symptoms because they are both caused by the rise in estrogen and progesterone that occurs before a period as well as after implantation.
  • You took the test too early, and there was not enough hCG (the pregnancy hormone) in your urine. Wait at least a week after your missed period when hCG levels are high enough to detect.
  • You are pregnant, but various complications produced a negative test result, such as having twins, an irregular menstrual cycle, or an ectopic pregnancy.
  • You are not pregnant, and the pregnancy-like symptoms you feel are a result of a false pregnancy (called pseudocyesis or phantom pregnancy).
  • You are not pregnant, but you have a condition or illness that mimics pregnancy symptoms (see below).

Am I Having a False Pregnancy?

According to infertility specialist Dr. Mark Trolice, "Pseudocyesis (false pregnancy) is a rare disorder in which the woman who is non-psychotic believes she is pregnant and experiences signs/symptoms despite a negative pregnancy test." This occurs when an intense desire for pregnancy tricks the body into thinking it is carrying a baby when it is not. The body will respond to this psychological desire by producing all of the typical pregnancy symptoms, such as a missed period, weight gain, nausea, tender breasts, sore nipples, or even fetal movement. Some women may only experience one of these symptoms at first and may have such a strong assumption she is pregnant that her mind tricks her endocrine system into producing more pregnancy-like symptoms. Your urine test should be negative if it is a false pregnancy, however, an ultrasound is the only guaranteed confirmation.

Conditions That Mimic Pregnancy Symptoms

Nausea ("Morning Sickness")

Feeling nauseous in the morning is not unique to pregnancy, so if you wake up wanting to vomit and a pregnancy test shows negative, you could be afflicted with low-blood sugar levels, insomnia, acid reflux, post-nasal drip, or anxiety/nervousness. Keep in mind that "morning sickness" a.k.a. pregnancy nausea actually happens throughout the day and is triggered by a sensitivity to certain smells and tastes. Nausea caused by other illnesses are not food-related, but rather a sign that there is something wrong in the digestive tract or with one's equilibrium.

Tender Breasts

Swollen breasts are one of many symptoms that are common to both pregnancy and PMS. Because of this, having achy breasts or sore nipples is not a good indicator of pregnancy. Certain medications, such as birth-control pills or hypothyroidism medication can increase the production of estrogen, causing your breasts to respond in the same way they would if estrogen levels rise before your period or in early pregnancy. If you are taking medication that may have an effect on your hormones and your pregnancy test shows negative, then you are not pregnant.

Missed Period

Women with irregular cycles or PCOS (polycystic ovarian syndrome) often miss their periods because of hormonal imbalance—some may even go months without a period if they have a severe imbalance. However, even women with regular menstrual cycles can miss a period or two due to stress, injury, weight gain, birth control, or any other anxiety-inducing reason that puts stress on the body and affects normal hormonal production.

Cramping and Spotting

Cramping and spotting a week before your period is usually a good indication of implantation, however, implantation bleeding and cramping may be confused for either early PMS cramping and spotting or ovulation. Ovulation, otherwise known as "mittelschmerz," occurs mid-cycle (about 10 days before your period) when the release of an egg might cause a sharp pinch or twinge on one side of the lower abdomen. Unlike implantation cramping (which lasts 1-3 days), ovulation cramping should only last a few hours at most.

Cramping and spotting could also occur if you have just started new birth control, you have uterine fibroids, or you have a hormonal imbalance (polycystic ovarian syndrome).

Fatigue

Feeling lethargic might be common in the first trimester of pregnancy, but it is also very common for a host of other illnesses and conditions, such as anemia, hyperthyroidism, kidney or liver diseases, obesity, etc. Feeling tired can also be a symptom of less serious causes, such as lack of sleep, having a cold or flu, or other lifestyle or diet changes that drain your energy supply. If you are suddenly feeling wiped out for no reason, but a pregnancy test shows negative, see a doctor right away to rule out any serious medical causes.

Mood Swings

Feeling emotional and antsy is another symptom that is common to both PMS and pregnancy. It could also be triggered by depression, hormonal imbalance, stress, anxiety, or other psychological conditions. Again, if you experience a sudden change in your emotional stability when you are usually in a good mood, check with the doctor to rule out any serious causes.

Constipation

Nearly half of pregnant women experience constipation during pregnancy because the rise in progesterone relaxes the digestive tract, which means food travels more slowly through the intestines. This symptom is not a good indicator of pregnancy alone, and if you're not pregnant, but you've been constipated, see a doctor right away. You could just need a boost of fiber, or it may be an early indication of serious gastrointestinal issues, such as colon cancer.

Frequent Urination

Urinary tract infection (UTI), overactive bladder, diabetes, incontinence, bladder stones, or ovarian cancer may be causes for why you feel like you constantly need to pee. However, this symptom is also common with PMS or menopause.

Weight Gain

Hormonal havoc is likely the cause if you are not pregnant. Women who suffer from polycystic ovarian syndrome (PCOS) often gain a lot of weight as a result of a hormonal imbalance in the adrenal glands or the ovaries. A hormonal imbalance can also lead to diabetes and other conditions that also increase a woman's weight and body fat. If you notice accompanying symptoms, such as hair growth or missed periods, then you likely need to get a hormone test to see which hormones you have an excess of and how to alleviate the hormonal chaos.

Illnesses With Pregnancy-Like Symptoms

Stress and Anxiety

Nausea is a common symptom of stress or anxiety. Anxiety can cause stomach aches, cramping, and even vomiting. If you have these symptoms accompanied by anxiety, you might consider seeking professional help for relief of stress or anxiety.

Ovulation and Premenstrual Symptoms

Some women get distinctive symptoms a few days before and during ovulation or a few days before a period. Pregnancy symptoms (nausea, cramps, bloating, tender breasts, etc.) are often similar to the symptoms of ovulation and menstruation.

Polycystic Ovarian Syndrome

4% to 12% of women suffer from PCOS, which greatly affects their menstrual cycle and fertility. Hormones in women with PCOS can range from excess testosterone to excess estrogen and progesterone. If you suffer from the latter, your body will exhibit symptoms that are similar to pregnancy. The most common symptoms of PCOS are weight gain (especially around the waist), pelvic pain, anxiety or depression, spotting outside of period, missed or late periods, and sore breasts. Ovarian Cysts

According to Dr. Carolyn Alexander who is board-certified in both Obstetrics and Gynecology as well as Reproductive Endocrinology, "An ovarian cyst can produce estrogen and progesterone and mimic early pregnancy symptoms such as nausea, bloating, frequent urination, mood swings, breast tenderness, cramping, pelvic discomfort." Ovarian cysts should be diagnosed by your OB/GYN with an ultrasound.

The Flu

Feelings of nausea or exhaustion can be caused by the common cold, flu, or other illnesses. You should talk to your healthcare provider if these symptoms linger for weeks.

Hormonal Contraception/Birth Control

The pill and other hormonal birth control methods commonly cause nausea, dizziness, breast tenderness, and mood changes. Try changing brands or methods if these side effects are troubling you.

Other Unknown Factors

If your symptoms are not explained by pregnancy and endure for weeks, you should see your doctor. You might need a blood test to test for abnormal thyroid functions or low hemoglobin count.

If You Still Think You're Pregnant...

  • Did you use the first urine in the morning? It has the highest hCG levels. If you didn't do the test first thing in the morning, your result may be inaccurate.
  • Did the test go properly? Did you see the control line or another indicator showing that the test worked as it should have? Read the instructions again to make sure it worked properly.
  • Did you try another type of test? Different brands of pregnancy tests all have different levels of sensitivity for hCG levels. You may want to try another brand to check your results.
  • Did you wait until at least 14 days after conception/ovulation? Before that the hCG levels are too low to be detected with a home test.

Could It Be a False Negative?

Getting a negative pregnancy test doesn't necessarily mean you're not pregnant. Sometimes tests are faulty or your testing methods are inaccurate (you tested too early, drank too much water, etc). There is also the chance that you are too far along in your pregnancy for the at-home test to detect.

How Soon Can You Take a Pregnancy Test

The sensitivity of each pregnancy test is variable, but the soonest the most sensitive test can detect a pregnancy is at least 12 days after conception. Conception happens around the time a woman ovulates and, in a typical 28-day cycle, ovulation happens on or around day 14. This means that if you have a 28-day cycle, the earliest time you can take a test and get an accurate result is four weeks after the first day of your last period.

Long and Irregular Cycles

Many women have long or irregular cycles. If you have not been carefully tracking your ovulation cycles, or if you can't predict it, it is hard to say when the earliest possible time is for an accurate result is. For women with long or irregular cycles, or if you don't know exactly what your cycle is, it is best to wait for the day the next cycle is supposed to begin (the day you expect to get your next period) to take the home test.

Wait a Week

It's hard to play the waiting game, but try to wait for a week to see if your period starts. If not, take a new test.

Hormonal fluctuations will cause these symptoms in women. There are 3 likely scenarios for experiencing nausea and sore breasts and getting a negative pregnancy test:

  • You are pregnant, but the test is inaccurate. Wait at least a week after your missed period to retake the test and test your urine first thing in the morning.
  • You are experiencing premenstrual symptoms. PMS symptoms can be confused with early pregnancy symptoms because they are almost identical.
  • You have a condition that causes hormonal imbalance, such as PCOS, hyper- or hypothyrodism, stress, or ovarian cancer.

Can Stress Cause False Pregnancy Symptoms?

Yes. Both psychological and physical issues can result in pseudocyesis, a false pregnancy. Stress is one of the main psychological factors that disrupts the body's hormonal balance. When you're stressed, your body releases the stress hormone, cortisol, which puts pressure on your adrenal glands and leads to an overproduction of androgens. This disruption triggers symptoms such as acne, sore breasts, missed periods, fatigue, and nausea—all symptoms that are similar to pregnancy.

How Common Is a False Pregnancy?

It only occurs in 1 to 2 women out of 22,000. Thus, it is extremely rare. Of the cases that do occur, they are predominately located in third world countries where childbearing is a necessary duty expected of all women. The pressure to produce children coupled with the lack of opportunities in other avenues of life lead to emotional stress, anxiety, and fantasies of pregnancy, all of which may result in pregnancy-like symptoms.

How to Stop a Phantom Pregnancy

According to Dr. Mark Trolice, an endocrinologist and infertility specialist, "Recovery may occur naturally, but women may experience labor pains prior to resolution." The best way to treat a false pregnancy is to confirm that you are not pregnant with an ultrasound and blood test. If you frequently experience pregnancy symptoms but you are otherwise healthy, then you may require counseling to gain perspective on why the disorder may be present.

But heres the thing only certain ones do. Theres also non-family planing clinics that do abortions. So theres the help i can offer on that.

Anyways im 5 days late, but I have had spotting. I'm 37 and used the diva/moon cup cause my husband and I have been trying for years. Because of the drive to get a sperm test done he hasn't been tested. I came up negative on the ovulation testing. But then I bought some home ones and passed a few, still no luck. So i did the baby dance and tried the diva cup to keep them in there. So im late been spotting but negative on the pregnancy test.

Now 16 years ago I had spotting but im not sure if it was implanting or like this because i wasn't trying to concive.

Anyways I guess I'll wait a week and if I still havent had a normal period I'll see a doctor. So fustrating. Has anyone tried this diva cup method? Anyone else end up pregnant after a negative pregnancy test?

As expected i missed my period and tested HMPT in 5 days post missed period it was negative so we went to Doctor for check up she did blood test and the HCG level was 10 %, and she asked me to come after one week means @ 12 days post missed period still in HMPT it's negative but this she didn't took blood test. but by Scan she said uterus size increased but not seems like baby tissue. she didn't took blood test. She said I'm not pregnant. Today I'm 15 days missed period I'm planning to wait one more week. Do i need wait ? But still i've frequent urination, food cravings, minor cramps, fatigue all these symptoms (not with Brest). Please do let me know do i need to wait or can i start hormone pills for periods ?

My period comes normally in 28 days,just 24th January I had my period but was abnormal to me cos it came lightly and ended early in one and a half day instead of three to four days.After that am still having abdominal pains,my stomach is heavy than usual,I also feel dizzness,sometimes lost of appetite and nausea.I used preg test strips it was negative.i had sex on my fertile period into my ovulation.Can it be possible am pregnant?

Hi, I had my last depo shot on 3sep 2018. In Nov my period came on the 1's, it went on and off until the third week. It didn't come on 1's december and 1's January. On the 19th december I took a test which came out negative. Yesterday 10 december another test came out negative. But now, on Friday 4January my uterus almost came out,I've had terrible pain like period pains ever since that day but do not have any sign of blood. The uterus feels like its back to its place but the cramps are still there. I have almost all the signs of pregnancy but I'm not convinced since I tested negative. I am scared to take pain killers and yet I don't have money for ultrasound. Sometimes I toss and turn with this pain, Yet again sex is not painful. What could be wrong?

So I was recently diagnosed with PCOS and my doctor told me to take birth control pills but to wait for my period since its due soon as I am at the secretory phase of my cycle (as seen in an ultrasound). Its been three weeks and no period. I have been showing signs of early pregnancy or just my period so Im not really sure. I did have sex a few days after my appointment and im worried of a condom failure. I tested a few days ago (so almost three weeks after sex) and it came out negative.

Did i test too early? Could I be pregnant? Or did I just skip my period because of my PCOS? Can you skip a period when youre already on the secretory phase of the cycle? If I did just skip, then what are these “signs” Im feelinf? Is it possible to have phantom pregnancy when you really dont want to be pregnant?

Hi, i just stopped with my "period" today. Usually before my period (but not every month) my breasts are tender. My breasts were tender 2 weeks before my period, during and after my period. I have lower back pain and i feel more tired than usual. We just came from holiday and i was well rested. I also have a weird bloated feeling to the stomach. I did not do a pregnancy test as i did have my period. But the flow was way lighter than it normally is. do you think i should do a pregnancy test that i might be pregnant.

You should be getting a possitive test if it's been 3 months since you last period, if the test on Monday is negative you can be quite sure. Maybe you should ask for a scan to see better what's going on. It might be PCOS for example, they can check for that in the scan. If you're not pregnant and they don't see anything in the scan, it's time to look at other causes like stress, anxiety, back problems or digestive problems.

This sounds more like it is the days before your period, some of the symptoms are similar. You should wait at least á week from now before taking another test if your period does not come in that time.

I have taken 3 home tests and an urine test at the dictors amd all negative. I have felt symptoms fir 2-3 months now.

Pain in lower and middle back, cramping in abdomen, fatigue, loss of concentration, frequent urination, mood swings, belly is bigger but weight is about the same, cant get comfortable at night, heightened sense of smell, acid reflux, constipation, nausea, loss of appetite, and i eat and stomach still rumbles and growls.

I dont feel like myself and go in for a blood test monday morning. A pregnancy test and other lab tests will be ran. I feel pregnant but all the tests shiw negative. I dint know whats going on.

Your symptoms are most likely the results of the shot rather then pregnancy symptoms. These symptoms are quite common in the fyrst 2-3 months on depo-provera.

So, I got a depo shot in april and in juky after the three months I got the paragard iud so i didnt have my period for 4 months. On september 7th I got I believe was my period but on the 8th I completely stopped bleeding and on the 9th I started bleeding again very light and on the 10th the bleeding was very havy and the 11th was the last day of what i think was my bleeding. My boyfriend and I have been trying to conceive our first baby for about 2-2/12 months. I know that window is very small and doesn't leave much time however, last month, I was exhibiting ALL of the signs. I even went to have a urinalysis done at a free clinic and it came up negative. I was seven days late on my period, cramping, tender breasts, etc however, it was negative. I am exhibiting the signs again this month however, I have to wait a bit longer to take the test since my expected ovulation date was last week on the 17th. I do not know what else to do aside for continuing to try and have a baby however, I do not want to continue to have my heart broken and his as well by all the negative tests. I’m in great hope to get some help and questions answered. Last month June 13th I started my period a week early and it only lasted for 3 days but it was really heavy which is abnormal for me. Since June I’ve been lightheaded, dizzy, nauseous, very fatigued, and constant headaches. My breasts have been a little more tender than usual. I had intercourse on the Friday June 2nd which was 3 days after my period. I really didn’t think anything of it since I got a period two weeks later but now I just got a period again extremely heavy Sunday July 8th. It was so much that I bled thru my pants and later that day I felt really dizzy and nauseated then started to sweat. It only lasted around 10mins. I went home and napped a few hours. When I woke up I still felt lightheaded and very very nauseated that I threw up which I never do. I took a pregnancy test and it came back negative so I’m not really understanding if I could possibly be pregnant and a test isn’t detecting it, maybe going thru a miscarriage, or there is some other problem with me that I’m not catching yet.

I stopped my birth control on the 6th of May had intercourse that same day. But even at the end of the month of April I was having diarrhea for 2 days. I got my period on the 8th of May for 2 days. 17th of May I felt a pulling cramp on my lower right abdomen and lower backpain for a few minutes. Later in the night I bleed little amount of red old blood and brown discharge last me for 2 days with minimal bleeding and no cramps after. The week of 21st of May felt dizzy and lightheaded and nausea and I feel hungry if I don't eat or drink. Therefore up until 17th of June no period seen. I did 2 hpt came out negative. Still feel lightheaded and nausea when eating sometimes no appetite. And emotional at times.

I'm now 2.5 weeks late for my period.. I've got bloating, back ache, loads of headaches and now thrush (which I've never had before)... however J keep getting negative pregnancy tests. We have been trying for a baby for a while now so I just don't know if this could be something or is just my body playing tricks. Is it common to get negative tests this far after a missed period?

Given the possibility of late ovulation, I would give it a week more, I would say that you have a window of a positive test until the 15th of June. After that, there are probably other reasons for your symptoms.

I just turned 40 in December, I have 24 day cycles and very regular. My last period was 4/30, should have started 5/24. Period has never come, so I'm about 2 weeks late. Do not take birth control and my partner and I have always said if it happens, great. I do keep track of ovulation and periods. We had unprotected sex in the last ovulation window. My previous pregnancy showed no signs of being pregnant except a missed period. I have been pregnant 6 times with only one birth. This missed period has many different things going on with it. A feeling of fullness in the pelvic area with tightness of jeans, a lot of white to clear egg white discharge, headaches, frequent urinating when I normally pee 3 times a day, some but not much nausea, gas, horrible fatigue. I have taken 2 hpt, one on 5/30 the other 6/2, both negative. I just recently started metformin for weight loss. I can't figure out what is going on. Should I make an appointment or just wait and see if AF shows up?

Hello! I had a withdrawal bleed (from stopping birth control pills) from April 12th to April 15th. Since then, I haven’t had a period at all. Last Tuesday, I had what I think was implantation bleeding. When I wiped, I saw very light pink...so light, I thought my eyes were playing tricks on me. Nothing after that. Was the pink discharge likely implantation? I tried two dollar store tests, but they were both negative. I definitely feel pregnant (extreme bloating, sore nipples, peeing more frequently, constipation, nausea which is more extreme when hungry.) Should I maybe try a different brand of test, or should I just wait another week? I have an OBGYN appt on the 20th, but I’d like to know now!

I started metformin a few months ago for weight loss, have very regular periods. My last period was 4/30-5/3. This cycle was supposed to start 5/24 (24 day cycles), I have taken two hpt, both negative. I have had horrible fatigue, urinating almost every hour during the day and I used to go about 3 times a day, feeling a fullness in the pubic area. I just turned 40 in December, my partner and I of 4 years do not use bc at all. Going on 2 weeks late

Please see a doktor as soon as possible to rule our ectopic pregnancy. If you can't get an appointment try an emergency room or walk in service, today.

If your cycle is regular, you should be getting a positive test by now. Taking into consideration that you could have had a late ovulation, you should be having a positive test during this week. If still negative in 4-5 days, I would guess that you are not pregnant and that there are some other reasons for you being late.
